Please read the information below before participating in TODAY'S ACTIVITIES.

I understand and am aware aerobic exercise, including the use of equipment is a potentially hazardous activity and that risk of injury may occur. I am voluntarily participating in these activities and expressly assume and accept any and all risk of injury.

 

I do hereby on behalf of myself and all others in my care I brought today to waive, release, and forever discharge GLAMOUR GIRL FITNESS/AND THE DUCHESS EVENT VENUE their officers, agents representatives, executors, relatives, executors, spouse, and all others from any and all responsibilities or liability from injuries or damages resulting from my participation in today’s activities including those caused by the negligent act or omission of any of those acting on their behalf or in any way arising out of or connected with my participation.

 

I do declare myself to be physically sound and suffering from no condition, impairment, disease, infirmity, or illness that would prevent my participation in the above event, and if I am aware of any of these stated issues I am not to participate in these exercise activities.
